Transfer of Patient Records

From: Dr. Trilochan (Terry) Papneja

I am writing to you in my capacity as the principal of Papneja Dentistry Professional Corporation, which formerly operated as “AXIS Dental” from 14 Lisa Street, Brampton, Ontario (the “Dental Practice”). This notice is to all present and past patients in respect of whom AXIS Dental maintains dental records (“Patient Records”). This includes all patients who were treated by any one or more of the following professionals:

Effective as of March 31, 2023 (the “Effective Date”), all Patient Records were transferred to Dr. Pierre Vigneault Dentistry Professional Corporation (now operating as “AXIS Dental” from the same premises at 14 Lisa Street, Brampton). Accordingly, as of the Effective Date:

1. Papneja Dentistry Professional Corporation ceased to be; and
2. Dr. Pierre Vigneault Dentistry Professional Corporation became,

the “health information custodian”, of all Patient Records for the purposes of the law that governs personal health information in Ontario, , the Personal Health Information Protection Act, 2004 (“Health Privacy Law”).

 I would like to assure you that:

• The practice continues to be operated as “AXIS Dental” from the same location in Brampton.
• The staff of AXIS Dental remains the same, with each of the above professionals continuing to provide services at AXIS Dental.
• The Patient Records were transferred by AXIS Dental to Dr. Pierre Vigneault Dentistry Professional Corporation in a secure manner and otherwise in compliance with the Health Privacy Law.  

If you have any questions regarding the transfer of Patient Records, you may contact me via the email address provided at the end of this letter, below my signature.

However, going forward, patients wishing to:

• access their Patient Records;
• request corrections to their Patient Records; and/or
• have their Patient Records transferred to another dentist or dental office,
